With an average daily census of 70-90 patients, a newly constructed state of the art free standing 8 bed inpatient unit and a widely recognized children’s bereavement program, East End Hospice serves the Long Island, New York, Eastern Suffolk County townships of Brookhaven, Riverhead, Southold, Shelter Island, Southampton and East Hampton.

Social Worker – Adult Bereavement Coordinator

East End Hospice is seeking an Social Worker Adult Bereavement Coordinator responsible for the overall management of a bereavement program. Provide bereavement services to East End Hospice families including but not limited to; individual and group counseling, crisis intervention, advocacy, information and referrals. 

As a member of the Interdisciplinary team, share in the assessment, admission process, preparation of the plan of treatment, care plan implementation, team review, care plan evaluation and revision with other members of the team.

Coordinate and implement the Community Bereavement Programs. Oversee the coordination and implementation of bereavement training and educational programs for East End Hospice staff and volunteers.  Assist with the planning and implementation of Camp Good Grief.

Supervise, instruct, guide and support social work bereavement personnel to provide high quality bereavement services to patients and families throughout all levels of care. Assist in the identification of psycho/social stressors of grieving patients and families.  Guide and support volunteer bereavement personnel to provide high quality bereavement services to families and loved ones.

QUALIFICATIONS:

Must be familiar with the hospice concept and its implementation.

Proficiency in the treatment of grief, bereavement, emotional trauma and facilitating the group process.

Ability to observe and assess the needs of the terminally ill and their families.

Ability to function as a member of an Interdisciplinary Team and promote effective interaction between team members.

PROFESSIONAL REQUIREMENTS:

Requirements include LCSW license and current registration to practice social work in the State of New York.

A master’s degree in social work from a graduate school of social work accredited by the Council on Social Work Education and the education department.

A minimum of five years of advanced practice in psychotherapy, death & dying, and grief.

Current NY Driver’s license and insurance.
