Positions Available: 

Field RN – Full Time and Part Time

East End Hospice is seeking a Full-Time Registered Nurse to join its Interdisciplinary care team serving patients at home and in other care settings in the community.

Responsibilities:

  • Teach, supervise, and counsel the patient and family regarding care and other issues related to the patient illness, including pain and symptom management.
  • Provide anticipatory guidance to patients/families to assist them in preparing for and coping with the events as they occur throughout the disease process.
  • Implement the nursing portion of the plan of care in coordination with the other members of the interdisciplinary team.

Qualifications:

  • Ability to relate well to terminally ill patients and their families.
  • Ability to assess patient/family needs and to be sensitive to those needs.
  • Willingness to function as a member of the interdisciplinary team.
  • Be familiar with the hospice concept and related nursing issues.

Requirements:

  • A registered professional nurse from an accredited nursing program. Currently licensed to practice nursing in New York State.
  • 2+ years of RN experience (Hospice, Med-Surg, Homecare experience) required
  • BSN preferred
  • Experience with Electronic Medical Records a plus

Registered Nurse – On-Call, Full Time

East End Hospice is seeking an experienced Registered Nurse to provide evening/night, weekend and holiday on-call nursing to its patients and families.

Schedule:

  • Two 12-hour weekend shifts, 9pm-9am, within a biweekly period.
  • Five 16-hour weekday shifts, 5pm-9am, within a biweekly period.

Responsibilities:

  • Triage telephone calls and respond to patient/family calls in a timely manner focusing on individual patient needs.
  • Perform emergent patient visits in the field (i.e., admissions, pronouncements), as needed.

  • Provide anticipatory guidance to patients/families to assist them in preparing for and coping with the events as they occur throughout the disease process.
